This form is used to check if you are listed on the National Driver Register. If you are listed, this file will help point you in the right direction so you can resolve the issues with your license and/or driving record by providing you with the a list of states that are currently prohibiting you from obtaining your drivers license.

2) National Driver Register Employee File Check Form

This form is used to check if a current or prospective employee is listed on the National Driver Register.  The following people may use this form:

  • Any individual under the provisions of the Privacy Act.
  • State and federal driver's license officials.
  • Current or prospective employers of motor vehicles operators.
  • Air Carriers for Pilot Applicants.
  • The Federal Railroad Administration and employers of railroad engineers.
  • The Federal Aviation Administration for airmen medical certification.
  • The U.S. Coast Guard for merchant mariner certification.
  • The National Transportation Safety Board.
  • Federal Highway Administration for Accident Investigations.
