The National Driver Register (NDR) is a computerized database of information about drivers who have had their licenses revoked or suspended, or who have been convicted of serious traffic violations such as driving while impaired by alcohol or drugs. State motor vehicle agencies provide NDR with the names of individuals who have lost their privilege or who have been convicted of a serious traffic violation. When a person applies for a driver's license the state checks to see if the name is on the NDR file. If a person has been reported to the National Driver Register as a problem driver, the license may be denied.

What Does This Mean For You?
This means that you need to first get your National Driver Register Driver Record File Check Form to request your NDR Driver Record. Then you will need to get your state driving record in order to know why your license has been revoked or suspended; and what actions need to be taken so that you can get your driver license privilages back.
This website was developed to assist the general public and employers with receiving the NDR national and state forms as quick as possible. In many instances, it can be very difficult and time consuming to obtain many of these forms on your own. Therefore, we have developed this website to make the process of finding these forms quick and easy for everyone. To recieve the forms from us, you must pay a nominal service fee.
The NDR currently does NOT have an automated system for receiving your National Driver Record. Therefore, you must complete the NDR National Request Form, have it notorized and mail it in to receive your record. Some states have created automated processes of requesting your record; while others still rely on mailing in a completed form.
