Obtaining a Copy of Your Driving History

Inquiries must be made on an Online Records Request (form J-23):

  • Check the box requesting Driving History.
  • Fill in section 1.
  • Complete the "requester" section at bottom of the form.  You must also include your telephone number.
  • Check made payable to "DMV" in the amount of $20.
  • Copies of two forms of identification from the requestor. (One form must be photo identification and the other form needs to also identify the requestor.)
  • Mail completed form and documents to:

In approximately one to two weeks you will receive a certified copy of your driving history.

  If you would like to obtain a copy of your own driver history, you may visit any

full service DMV Branch Office

.  You will need to provide your name, address, driver license number, date of birth, two forms of identification as stated on the

Online Records Request

 (form J-23) and the required fee of $20.
