National Driver Register Forms Website Mission Statement: To provide the service of helping the general public to get the National Driver Register forms that are needed in order to check both their National Driving Record and their Local/State Driving Record saving the public time and frustration.
There are two ways to get your Illinois Driver Record:
In person - This is the quickest way to obtain an abstract of your driving record. Complete the Illinois Record Request Form from this website and bring it to your nearest Secretary of State facility. You will need to pay a $12 fee to the State of Illinois (no credit or debit cards accepted) and receive the record on the spot.
Mail - A second option is to mail your request to the Secretary of State, Driver Analysis Section, 2701 S. Dirksen Parkway, Springfield, Il 62723. You will need to include the full name, date of birth, sex, and Illinois driver's license if possible along with your $12 fee. Please use the abstract request form from this website. Please allow 10 working days for a response.
