Your driving history record is available from the Massachusetts Merit Rating Board.
The Merit Rating Board (MRB) is responsible for maintaining a driving history record of surchargeable incidents for Massachusetts drivers. You can request a copy of your driving history record at any time by submitting a request form to the MRB. There is a $15 fee to process your request.
What’s on your record?
Your driving history record reports, by surcharge date, any surchargeable incident that occurred within the six years prior to the date your request is processed. For example, if we process your request on July 6, 2008, your record will include all incidents with surcharge dates between July 6, 2002, and July 6, 2008.

Your request form will be returned to you if we cannot validate your operator information. Make sure your driver’s license number, date of birth, and name exactly match the information on the front of your Massachusetts Driver’s License.

The $15 processing fee must be included with your request. This fee is non-refundable. |

Personal checks or money orders are accepted. Do not mail cash with your request. |

Make your check or money order payable to Commonwealth of Massachusetts. Remember to sign your check or money order. |

If your mailing address is different from the address shown on your driver’s license, be sure to provide this information. We will mail your record to your mailing address. |
