Motor Vehicle Reports (Driving Records)

A request for a Motor Vehicle Report (MVR), also known as a driving record, must be accompanied by a Records Request form. If you are requesting an MVR for someone other than yourself, you must also provide the Department with a Consent to Release form. Also, please include a self-addressed stamped envelope with your request.  Both the Records Request Form and the Consent to Release Form are available from this website with only paying one service fee.

The MVR contains all entries related to the person's driving record for three (3) years prior to the date of request, as required by Oklahoma Statute. MVRs are also available from motor license agents (also known as "Tag Agents") throughout the state. NOTE: Certified copies are available only from the Department.

The MVR is available for:
$13 per certified record, or
$10 per uncertified record.
(NOTE: the Department does not accept personal checks for driving records.)
