The following request forms are required on behalf of the South Dakota Department of Public Safety to ensure compliance with the provisions of the Drivers Privacy Protection Act to allow the release of motor vehicle records to you or your company. Please complete the form appropriate to you and have the form notarized. Return the form along with the fee (if applicable) of $4.00.
