Fee: $5.00 (Government agencies are exempt from payment)
In Person: Present a Wyoming driver license or Identification Card, present proof of identification.
By Mail: Wyoming Department of Transportation Driver Services - Driving Records, 5300 Bishop Boulevard, Cheyenne, WY 82009-3340. Please allow 7-10 working days to process your request.
Download driving record form (DSFR-11E) OR send a written request which includes your full name, date of birth, Wyoming driver license number or social security number. All requests must include the $5.00 fee - check or money order payable to WYDOT. Please allow 7-10 working days to process your request.
